Purpose:
- To honor individuals who have made
an outstanding contribution to the state association through
time or task oriented activities.
- To instill in members a desire to contribute to the growth
and development of the state association outside of serving
as an elected officer.
Awarding Procedures:
- Awards to be announced at the annual meeting,
- Recipients will be presented with the citation highlighting
the service of the individual.
- Recipients name will be engraved on a permanent plaque.

knowledge in clinical practice to advance the development and
awareness of OT

PURPOSE:
- To promote AOTA Annual Conference attendance of VOTA members.
PROCESS:
- Applications must be postmarked by December 31st of the calendar year preceding the conference
- Applications will be reviewed by the Board at the board meeting following receipt of the application
- Checks will be mailed to scholarship recipients once the Board has received proof of attendance (a copy of certificate of attendance)
- Scholarship recipients will provide a 150 word submission to the VOTA Newsletter within 2 months following the AOTA Annual Conference
- Scholarship recipients will be invited to present at a Board meeting and at annual conference
- Scholarships are awarded at the discretion of the Board
- Board members are not eligible for the scholarship
- Award recipients are not eligible for 2 years following receipt of the scholarship

PURPOSE:
To recognize an OT/OTA member for their contribution to the advancement
of the Occupational Therapy
profession.
SELECTION CRITERIA:
The nominee shall be an OT or an OTA who is currently or has
been a member of VOTA. The nominee
shall have made a significant contribution to the profession of
Occupational Therapy in one of the
following areas:
- Therapeutic practice, technique, modality
- Program/service development
- Education
- Legislation
- Public relations/promotion of OT, recruitment
- Community
- Management
The nominee shall have practiced the profession of Occupational
Therapy for at least 4 years.
NOMINATION AND AWARDING PROCEDURES:
- The VOTA membership nominates the candidates
- Nominees for this
award can be considered each year.
- One nominee will be selected
each year. Individuals who are nominated and not selected may
be renominated again at a later date. The nomination form must
be completed and submitted each time.
- The VOTA executive board
reviews nominees and selects based on information submitted which
demonstrates the nominee meets the established selection criteria.
- A
descriptive statement is written by the nominator, which provides
additional information to be used in
presenting the award to the selected candidate.
- The President
of VOTA shall present the selected candidate a certificate award
at the annual VOTA conference
- The name of the awarded shall be
published in the VOTA Newsletter, OT Practice and announcement
posted in the Burlington Free Press and/or the recipient’s
local newspaper.
